PENGGUNAAN KORTIKOSTEROID SEBAGAI BRIDGING THERAPHY PADA RHEUMATOID ARTHRITIS: LITERATURE REVIEW

Abstract

Rheumatoid arthritis merupakan penyakit autoimun kronis yang menyebabkan inflamasi pada sendi-sendi perifer, terutama tangan dan kaki, yang dapat menimbulkan deformitas serta disabilitas bila tidak ditangani secara dini. Penyakit ini bersifat multifaktorial, melibatkan faktor genetik, lingkungan, dan disregulasi sistem imun yang memicu pembentukan autoantibodi seperti Rheumatoid Factor (RF) dan Anti-Citrullinated Protein Antibodies (ACPA). Terapi utama RA adalah Disease Modifying Antirheumatic Drugs (DMARDs), namun efek terapeutiknya baru muncul setelah beberapa minggu hingga bulan. Oleh karena itu, kortikosteroid digunakan sebagai bridging therapy untuk mengontrol inflamasi sementara menunggu efek DMARDs bekerja. Kombinasi kortikosteroid dengan DMARDs efektif menurunkan aktivitas penyakit, mengurangi nyeri sendi, serta memperbaiki kadar biomarker inflamasi seperti C-Reactive Protein (CRP) dan Laju Endap Darah (LED). Namun, penggunaan kortikosteroid perlu dibatasi karena berpotensi menimbulkan efek samping, termasuk peningkatan risiko infeksi, gangguan metabolik, osteoporosis, serta insufisiensi adrenal bila digunakan jangka panjang. Oleh sebab itu, terapi ini harus diberikan dengan prinsip lowest effective dose dan shortest possible duration disertai pemantauan ketat. Penggunaan kortikosteroid secara rasional sebagai bridging therapy terbukti memberikan manfaat signifikan dalam pengendalian gejala dan aktivitas penyakit pada pasien RA.
