FARMAKOKINETIK TENOFOVIR DISOPROFIL FUMARATE DAN TENOFOVIR ALAFENAMIDE PADA PASIEN HEPATITIS B: SYSTEMATIC REVIEW

Abstract

Hepatitis B merupakan masalah kesehatan global yang serius dengan sekitar 296 juta penderita di seluruh dunia. Tenofovir merupakan lini pertama dalam pengobatan hepatitis B, namun memiliki permeabilitas membran yang buruk dan bioavailabilitas oral rendah sehingga dikembangkan dalam bentuk prodrug yaitu Tenofovir Disoproxil Fumarat (TDF) dan Tenofovir Alafenamide (TAF). Penelitian ini bertujuan untuk menganalisis dan membandingkan profil farmakokinetika TDF dan TAF pada pasien hepatitis B melalui pendekatan systematic review. Metode penelusuran artikel dilakukan melalui database PubMed dan Google Scholar dalam rentang waktu 20 tahun terakhir dengan kata kunci spesifik farmakokinetik, tenofovir, TDF, TAF dan hepatitis B. Dari 217 artikel yang ditemukan, 3 artikel memenuhi kriteria inklusi untuk ditelaah. Hasil menunjukkan TAF memiliki stabilitas plasma yang lebih baik sehingga dapat mengirimkan metabolit aktif lebih efisien ke hepatosit dengan konsentrasi intraseluler yang lebih tinggi pada dosis yang lebih rendah dibandingkan TDF. Konsentrasi sistemik TAF yang lebih rendah juga mengurangi risiko efek samping terutama nefrotoksisitas. Dapat disimpulkan bahwa TAF memiliki profil farmakokinetika yang lebih menguntungkan dibandingkan TDF dalam pengobatan hepatitis B.
