IMPLEMENTASI AMBULASI UNTUK PENCEGAHAN DEKUBITUS PADA PASIEN STROKE HEMORAGIK DI RUMAH SAKIT TK II PUTRI HIJAU MEDAN

Abstract

Hemorrhagic stroke is a medical condition that causes decreased mobility due to bleeding in the brain, leading patients to experience prolonged bed rest. This condition increases the risk of complications, particularly impaired skin integrity in the form of pressure ulcers. One important intervention to prevent pressure ulcers is early ambulation, which involves regularly changing body positions. This study aims to describe the implementation of ambulation as an effort to prevent pressure ulcers in patients with hemorrhagic stroke at TK II Putri Hijau Hospital Medan. This research is a descriptive study with a case study approach on two patients with hemorrhagic stroke. The case study applied the medical-surgical nursing care process, including assessment, nursing diagnosis, planning, implementation of ambulation actions such as turning to the right and left every 2 hours for 3 consecutive days, and evaluation of the interventions provided. The results showed an increase in muscle strength from a score of 1/5 to 3/5. The skin remained clean and intact, indicating that ambulation was effective in maintaining skin integrity. The conclusion of this study is that regular early ambulation can prevent pressure ulcers and improve muscle strength in immobilized patients with hemorrhagic stroke. This ambulation intervention has proven to provide positive impacts on maintaining skin integrity and improving the quality of patient care.
