EFEKTIVITAS LARUTAN NACL 0,9% TERHADAP PROSES PENYEMBUHAN LUKA ULKUS DIABETIKUM BERBASIS NARRATIVE LITERATURE REVIEW

Abstract

Ulkus kaki diabetik (DFU) adalah komplikasi yang umum dan serius pada diabetes mellitus yang sering mengakibatkan proses penyembuhan luka yang lebih lambat, infeksi, dan peningkatan risiko amputasi. Perawatan luka yang benar sangat krusial untuk mempercepat proses penyembuhan dan menghindari komplikasi, di mana penggunaan cairan irigasi berperan penting dalam menjaga kebersihan dan kelembapan luka. Larutan NaCl 0,9% banyak dipakai karena karakteristik isotonik, aman, tidak beracun, serta terjangkau. Tinjauan pustaka naratif ini bertujuan untuk menilai efektivitas larutan NaCl 0,9% dalam penyembuhan luka ulkus diabetikum dengan mensintesis temuan dari sepuluh penelitian komparatif, yang meliputi uji coba teracak, studi quasi-eksperimental, dan studi prospektif. Evaluasi menunjukkan bahwa meskipun NaCl 0,9% berfungsi sebagai antiseptik untuk membersihkan luka, efektivitas terapeutiknya terbatas, yang menyebabkan pengurangan ukuran luka lebih lambat, pembentukan jaringan granulasi yang kurang baik, dan peningkatan tingkat infeksi dibandingkan dengan agen topikal lainnya. Agen topikal seperti sucralfate, insulin, plasma kaya trombosit, perak nano-kristalin terionisasi, oktendidin dihidroklorida, metronidazol, saline hipertonik, dan Piper crocatium menunjukkan hasil yang lebih optimal dalam mempercepat penyembuhan luka, meningkatkan kualitas jaringan granulasi, serta mengendalikan infeksi. Hasil penelitian ini menunjukkan bahwa walaupun NaCl 0,9% tetap vital sebagai perawatan dasar yang aman dan terjangkau, penambahan agen topikal yang memiliki mekanisme aksi khusus dapat memperbaiki intervensi awal, meningkatkan hasil penyembuhan, dan mengurangi kemungkinan komplikasi pada pasien dengan DFU.
